About the Role

The Health Care Assistant position at CHCP focuses on supporting senior healthcare providers, including Care Leaders, Deputy Managers, and qualified nurses, in providing person-centered care. Working closely with the Intermediate Care Team, the role includes both day and night shifts to meet the needs of patients in the Residential Nursing Dementia and Rehabilitation Unit. The main responsibilities involve implementing patient care plans, ensuring a high standard of care, and collaborating with other Health Care Assistants to maintain effective operations within the facility.

Key Responsibilities

  • Assist Care Leaders and Deputy Managers in daily operations within the Residential and Rehabilitation Unit
  • Work collaboratively with a team of Health Care Assistants to deliver personalized patient care
  • Follow prescribed care plans to meet individual patient needs
  • Uphold best practices and contribute to an efficient, compassionate care environment

This position is integral to maintaining high standards of patient care and involves ongoing support from senior staff to promote growth and skill development.
